To start with I thought, I should post some thing which will help many of us in our day to day life in our profession.

Root cause analysis is one of the area we fail many times. I also come across examples where symptoms are treated as root causes and actions are taken on symptoms. This results in recurrence of defects and more money loss.

Root causes need to be addressed very effectively to counter defects. There are many ways root cause analysis can be carried out and there are no specific patterns. However, I find the below link will help you in getting the root cause analysis approach right.
